Documents Required for Application

If you are a resident currently registered in Chiba Prefecture

Japanese Nationals

  1. Valid passport
  2. Any of the following documents:
    • Valid driver's license
    • A Certificate of Residence issued within the last six months
    • A Certificate of Items Listed in Resident Register issued within the last six months
  3. Document which helps determine whether your circumstances qualify as grounds on which a certificate of criminal record can be issued (see note below)

Non-Japanese Nationals

  1. Valid passport
  2. Any of the following documents:
    • Document issued by a government or municipal office verifying your name and current address, such as Special Permanent Resident Certificate and Resident Card
  3. Document which helps determine whether your circumstances qualify as grounds on which a certificate of criminal record can be issued (see note below)
If you are residing abroad with the last registered residence in Chiba Prefecture

Japanese Nationals

  1. Valid passport
  2. Any of the following documents:
    • An Official Copy of Deleted Resident Register issued within the last six months
    • An Official Copy of Past Residential Record in Family Register issued within the last six months
  3. Document verifying your current address abroad such as your local driver’s license or mail addressed to you
  4. Document which helps determine whether your circumstances qualify as grounds on which a certificate of criminal record can be issued (see note below)

Non-Japanese Nationals

  1. Valid passport
  2. Document verifying your current address abroad such as your local driver’s license or mail addressed to you
  3. Document issued by a government or municipal office verifying your name and the last registered address in Japan
  4. Document which helps determine whether your circumstances qualify as grounds on which a certificate of criminal record can be issued (see note below)

Note: “Document which helps determine whether your circumstances qualify as grounds on which a certificate of criminal record can be issued” means a completed visa application form or a letter of request from an organization to which you are to submit the certificate.

※ You are advised to make inquiries beforehand as additional documents may be required.
※ If you are planning to have your passport renewed or have your name or other items changed on your passport, please do so before applying for a certificate of criminal record.
