Documents Required for Application

Application for Survivor Benefit
1 A document which proves the date of death of the victim
2 A certified copy of the family register that proves the applicant's relationship to the deceased victim
3 A Certificate of Residence
4 A document that proves that the daily life of the applicant depended upon the income of the deceased victim
5 A document that proves the amount of the per diem income of the deceased victim
6 A document that proves the amount of financial burden on the victim
7 A document that proves the number of lives taken
Application for Severe Injury or Disease Benefit
1 A medical certificate that validates a serious injury or illness suffered by the victim
2 A copy of the health insurance certificate of the victim
3 Documents which prove the amount of financial burden on the victim
4 A document that proves the number of lives taken
5 A document that proves the amount of income earned a day on average
Application for Disability Benefit
1 A medical certificate concerning the area and condition of disability of the victim
2 A document that proves the amount of the per diem income of the victim

Deadline for Application

You cannot make an application for benefit if two years have passed since the date you learned of the victim's death, severe injury or illness, or disability caused by a criminal act; or if seven years have passed since the date of the victim's death, severe injury or disability.
